    Please download and complete the authorization form. Alternatively, we will accept any form you have created, provided it includes all the information indicated on our authorization form.

    • No consents are required for CYFSA Proceedings.
    • Consents/Court Order required for all other requests.
    • The written consent of any person aged 16 years and older is required to release their information and must be included with your request. They can complete the Consent to Disclose Information form and you can attach it to your request.
    • A person with custody of a child under the age of 16 may request personal information of that child, subject to some exceptions. In some circumstances, you may be asked to provide documents confirming your authority to make the request in place of the child.

    Please be advised the following materials are vetted from the file:

    • “Record of Personal Health Information” as defined in the Mental Health Act;
    • “Records of a mental disorder as defined by section 294 of the Child, Youth and Family Services Act, (on the grounds that no physician has been consulted with respect to possible harm to the patient or other persons resulting from release of such records);
    • Any reference to registration of a person on the Child Abuse Registry, established under section 133 of the Child, Youth and Family Services Act ;
    • Any information which would identify third party informants who have provided information to the Society regarding risk to a child (on the basis of the public policy argument that identification of such parties who thought they could remain anonymous will result in fewer people reporting); Addresses and phone numbers of foster parents/volunteer drivers are also removed;
    • Any records/information relating to a young person’s involvement with the Young Offenders Act or the Youth Criminal Justice Act;
    • Notes regarding the Society workers’ discussions with legal counsel (on the grounds of solicitor-client privilege)
